Trip Forms
After SOAC confirms your child's enrollment in a Summer Trip, you will be mailed a packet with a booklet about the trip and four forms that must be filled out, signed and returned by May 17th, 2023. You may bring the completed forms to the Parent Meeting at 7pm that evening at Pineview Elementary in Reedsburg, or mail them back to 150 8th Street, Reedsburg, WI 53959.
